From entering our office for your complimentary consultation to your last appointment, you’ll feel the special spirit of A List Smiles Orthodontics. Fun, friendly and passionate about our patients, we strive to make every visit a memorable one. We want to be the highlight of your day!
Our team wants to ensure that every patient is provided with the proper amount of time to receive the care they deserve. In an effort to keep our wait times to a minimum, we ask that patients arrive 15 minutes prior to their first appointment time. If you are unable to keep your appointment we require a 24-hour cancellation notice.
New Patient Forms
Please find links to download your new patient paperwork. We ask that you bring your new patient paperwork with you to your first appointment, arriving 15 minutes early. Please bring a photo ID and insurance card or Medicaid card to each appointment.
Why the A List ?
Your first visit to our office will include an examination, x-rays, intra-oral photos, impressions and if needed, a treatment plan. Dr. Alyssa Sprowl has years of experience and knowledge in even the most complex cases. A List Smiles Orthodontics believes in open communication delivered through working closely within the team and if referred, your referring doctor, to develop a treatment plan that keeps your best interest and health as the number one priority.
